Download Address or Email or Mobile Number Modification Form or take first page from KYC form which is KRA form. Fill the information you want to change besides your Name and PAN number in the the KRA form, sign it and attach self certified PAN card copy.
You need not to attach any other additional document for change of Telephone Number, Mobile Number and or Email id besides mentioning in KRA form.
